Privacy Policy and Notice of Privacy Practices
How Dr. Jeremy Goodman’s telehealth practice handles website information and protects patient health information.
This page has two parts. Part 1 explains what this website (jeremygoodmanmd.com) collects. Part 2 is our Notice of Privacy Practices, which explains how we use and protect your health information once you become a patient of Goodman Longevity, Professional Corporation (the medical practice of Jeremy Goodman, MD).
Part 1. Website privacy
What this website collects
This website is for information and for requesting a visit. It is not part of your medical record, and it is not a secure place for health details.
- Request form: your name, email, mobile phone, the state you’ll be in for your visit, the general topics you choose, how you’d like us to reach you, an optional short note and your consent to be contacted.
- Contact form: your name, email, state, a topic and your message.
- Email: anything you choose to send to jeremy@goodmanlongevity.com.
Please don’t include medical details in the website forms or in email. We’ll collect those privately through the patient portal.
Booking, intake and payment
When you book, you are taken to our scheduling and patient portal provider, Healthie. Your intake forms, consent forms, visit notes and video visits take place there, not on this website. Healthie handles your health information under a HIPAA business associate agreement with the practice.
Payments are processed by the portal’s payment processor, Stripe. We do not store full card numbers. Payment descriptions are kept generic (for example, "Consultation").
Technical information and cookies
Our website host keeps standard server logs (such as IP address, browser type and pages requested) to run and secure the site.
At this time the website does not use advertising pixels or third-party analytics cookies. It uses only what it needs to work, plus the standard server logs our web host keeps.
Videos on this site are hosted by YouTube. A video loads from YouTube only when you press play, and YouTube’s own privacy policy then applies.
How we use website information
We use what you send us to respond to you, schedule your visit, operate and secure the site, and meet legal obligations. We do not sell your information, and we do not share it for targeted advertising.
If you ask us to text you, message and data rates may apply. You can reply STOP to opt out of texts at any time.
Who we share it with
We share website information only with service providers that help us run the site, receive form submissions, schedule visits and take payments, under confidentiality obligations, or when the law requires it.
Consumer health data (Washington, Nevada and other states)
Some states, including Washington and Nevada, have laws that protect "consumer health data," which can include the topics you pick in our request form. We collect that information only to respond to your request and schedule your care, we do not sell it, and you can ask us to see or delete it at any time by emailing jeremy@goodmanlongevity.com.
Your choices
You can ask us to access, correct or delete the contact information you sent through this website by emailing jeremy@goodmanlongevity.com. Residents of California and other states may have additional rights under state law. We will not treat you differently for using your privacy rights. Information that becomes part of your medical record is kept as required by medical record laws and is covered by Part 2.
Keeping information and security
We keep website form submissions only as long as needed to respond and schedule your visit, and no longer than 12 months unless the law requires otherwise. We use reasonable safeguards, but no website or email system is completely secure.
Children
This website and our telehealth services are for adults 18 and older.
Part 2. Notice of Privacy Practices
THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.
Our commitment
We keep your health information confidential as required by applicable federal and state law, including California’s Confidentiality of Medical Information Act, and we follow the practices in this notice. We will tell you promptly if a breach occurs that may have compromised the privacy or security of your information.
How we use and share your health information
- Treatment: for example, sending a prescription to a pharmacy, ordering labs, or coordinating with another doctor caring for you.
- Payment: for example, processing your payment for a visit.
- Running the practice: for example, scheduling, reminders, quality review, and working with vendors (such as our patient portal) who agree in writing to protect your information.
- When the law requires or allows it: public health and safety reporting (including prescription drug monitoring programs), health oversight, lawsuits and legal actions, law enforcement in limited cases, preventing a serious threat to health or safety, coroners, workers’ compensation, and specialized government functions.
Uses that need your written permission
We will not use or share your information for marketing, sell it, or use or share it for any purpose not described in this notice unless you give us written permission. You can take back that permission in writing at any time. Patient stories, photos or testimonials are never used without your written permission.
Your rights
- Get a copy of your medical record, including an electronic copy.
- Ask us to correct your record.
- Ask us to contact you in a specific way (for example, a certain phone or email) or at a different address.
- Ask us to limit what we use or share. We are not required to agree in all cases, but we must agree not to share information with a health plan about a service you paid for in full yourself, if you ask.
- Get a list of certain times we’ve shared your information.
- Get a paper copy of this notice at any time.
- Choose someone to act for you, such as a legal guardian or someone with medical power of attorney.
Complaints
If you believe your privacy rights have been violated, contact us by email at jeremy@goodmanlongevity.com. You may also file a complaint with the U.S. Department of Health and Human Services, Office for Civil Rights (hhs.gov/ocr/complaints), or with your state. We will not retaliate against you for filing a complaint.
Changes to this policy and notice
We may update this page. Changes apply to all information we hold. The effective date is shown at the top, and a current copy is always available here and on request.
