Terms of Use
Terms for using jeremygoodmanmd.com and booking telehealth visits with Dr. Jeremy Goodman.
These terms apply to your use of jeremygoodmanmd.com, operated by Goodman Longevity, Professional Corporation (the medical practice of Jeremy Goodman, MD). By using the site, you agree to them. If you don’t agree, please don’t use the site.
Not medical advice
Content on this site and in our videos is general education. It is not medical advice for your situation and is not a substitute for a visit with a qualified clinician.
When care begins
Using this site, sending a form, emailing us or booking a visit does not by itself create a doctor-patient relationship. That relationship begins when Jeremy Goodman, MD evaluates you at a visit and agrees to provide care.
Not for emergencies
Do not use this site, email, forms or text messages for urgent or emergency needs. If you have a medical emergency, call 911 or go to the nearest emergency room.
Who can use our telehealth services
- You must be 18 or older.
- You must be physically located in California, Georgia, Nevada, Washington, Ohio or Kentucky at the time of each telehealth visit.
- You must give accurate, complete information about your identity, location and health, and show a valid photo ID if asked.
Treatment is a medical decision
Any testing, prescription or treatment is decided by Jeremy Goodman, MD using his medical judgment. Booking or paying for a visit does not guarantee any prescription, including testosterone or other controlled medications. Federal and state rules may require an in-person exam or other steps before some medications can be prescribed.
Fees and payment
Telehealth visits are cash-pay. The current fees are $450 for a new telehealth consult and $250 for a telehealth follow-up. We do not bill insurance. Labs, medications and pharmacy charges are billed separately by those providers. Fees may change; the fee shown when you book applies to that visit. Cancellations and refunds follow our Cancellation & Refund Policy.
Third-party services
Scheduling, the patient portal, video visits, payment processing, labs and pharmacies are provided by third parties with their own terms and privacy policies.
Content and acceptable use
Site text, photos, videos and branding belong to the practice or its licensors and may not be copied or reused without permission. Don’t misuse the site, submit false information, or try to interfere with its security.
Links
Links to other sites, including YouTube, are for convenience. We are not responsible for their content or practices.
Disclaimers and limits on liability
The site is provided "as is." To the fullest extent the law allows, the practice is not liable for indirect or consequential damages arising from use of the website. Nothing in these terms limits any duty of care owed to you as a patient or any rights you have that cannot be waived by law.
Governing law
These terms are governed by California law, without regard to conflict-of-law rules, except where the law of the state where you receive care requires otherwise.
Changes
We may update these terms. The effective date at the top shows the latest version.
