Telehealth Informed Consent
What to know before a telehealth visit with Dr. Jeremy Goodman, including benefits, risks, prescribing limits and your rights.
Please read this before your first telehealth visit with Jeremy Goodman, MD. You will be asked to confirm your consent in the secure patient portal before your visit.
What telehealth is
Telehealth means receiving care by live, secure video (and, when appropriate, phone or secure messaging) instead of in person. Your visit is with Jeremy Goodman, MD, a board-certified urologist licensed in the state where you are located during the visit.
Where you must be
You must be physically located in California, Georgia, Nevada, Washington, Ohio or Kentucky at the time of each visit. We will confirm your identity and location at the start of each visit. If you are not in one of these states, we will reschedule.
Benefits
- Care from a specialist without travel or a waiting room.
- Easier follow-up and lab review.
- Privacy: you choose where you take the visit.
Risks and limits
- A video visit cannot replace a hands-on physical exam. Some conditions need an in-person exam, labs or imaging before a diagnosis or treatment.
- Poor video or audio, or a dropped connection, can delay or interrupt a visit.
- Although we use secure systems, there is a small risk that electronic information could be accessed without permission.
- Information you provide may be incomplete, which can affect the advice you receive.
Prescriptions
Jeremy Goodman, MD prescribes only when it is medically appropriate. A prescription is never guaranteed. Testosterone is a federally controlled substance, and federal and state laws may require an in-person exam, a check of your state’s prescription monitoring database, regular labs or other steps before or during treatment. Prescriptions are sent to a licensed U.S. pharmacy.
Your responsibilities
- Join from a private, safe place, and not while driving.
- Give complete and accurate information about your health, medications and location.
- Follow up as advised, including labs and any in-person care that is recommended.
- Do not record a visit without everyone’s agreement.
Your rights
- You may decline or stop telehealth at any time and ask about other options, including in-person care.
- You may ask questions at any time.
- Your records are kept confidential as described in our Privacy Policy and Notice of Privacy Practices, and you can request a copy.
Emergencies
Telehealth is not for emergencies. If you have a medical emergency, call 911 or go to the nearest emergency room.
Fees
Visits are cash-pay as described in our Terms of Use and Cancellation & Refund Policy.
Your consent
By confirming in the patient portal, you agree that you have read this information, have had the chance to ask questions, and consent to receive care by telehealth. Your consent is documented in your record before your first visit.
